Unfortunately, at the moment Hawkins Medical Clinic is not accepting new patients. This decision is likely to be reviewed early 2025.

Authority Form
Patients should complete this form and bring it into the clinic for actioning if they have anyone they would like to pick up scripts/test results on their behalf or enquire regarding their account.

Transfer of Medical Records Form
Patients transferring their care over to Hawkins Medical Clinic should complete this form so we can request a copy of your medical records.
